To refer a patient simply fill in the form below. Once we have received the referral form we will contact the patient directly and treat as requested. On completion of the referral request your patient will be returned to you for continual care. It is our strict policy that on completion of the referred treatment, no referred patient will be seen again without an express request from their referring dentist.
